29 CFR 1910.333(a)(2): Where exposed electrically charged parts were not de-energized safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work is performed near or on equipment or circuits which are or may be energized. The specific safety-related work practices shall be consistent with the nature and extent of the associated electrical hazards: (a) Fusion Water-Jet/Saw Cutting Machine - Serial No. 1777580: On or about March 22, 2017, an employee/machine operator did not employ electrical safety-related work practices while working on or around the exposed 480 Volt AC energized electrical components within the water-jet/saw electrical enclosure. This exposed the employee to electrical arc flash hazards and electrical shock hazards.
