29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. Larose Industries, LLC dba Cra-Z-Art - Fayette, Ohio: On or about November 30, 2016, employees were exposed to rotating part hazards while servicing the #P4 Resina screw capper machine (model U301008; s/n 74 5016). An employee sustained amputation injuries when unjamming bottles at the exit side of the conveyor during a time the machine was not shut down and locked out. The employer failed to ensure that energy control application steps to control the machine's electrical energy source were implemented prior to unjamming the machine; the employer did not: - Ensure employees had the method or means to control hazardous energy. - Ensure that equipment was turned off or shut down. - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Verify that isolation and deenergizeation of the machine or equipment had been accomplished. b. Larose Industries, LLC dba Cra-Z-Art - Fayette, Ohio: On or about February 7, 2017, employees were exposed to rotating part hazards in that the #P4 Resina screw capper machine (model U301008; s/n 74 5016) was not locked out prior to cleaning the machine. The employer did not ensure that the energy control application steps to control the machine's electrical energy source were implemented prior to the servicing task; the employer did not: - Ensure employees had the method or means to control hazardous energy. - Ensure that equipment was turned off or shut down. - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Verify that isolation and deenergizeation of the machine or equipment had been accomplished.
