29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed: (a) Servtech at 333 Centennial Pkwy. Louisville, CO: On or before 1/19/2017, the employer had not conducted periodic inspections of all energy control procedures including but not limited to the Drill-Line CNC hole cutting machine (Ocean Machinery, Model: MDL-1000/1C, Custom Avenger 40, SN: A002943), to ensure all energy sources capable of causing injury are isolated and rendered inoperative prior to servicing and or maintenance. This condition exposed employees to amputation hazards.
