29 CFR 1926.416(a)(1): The employer permitted an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, were as the employee was not protected against electric shock by de-energizing the circuit and grounding it or by guarding it effectively by insulation or other means: a) Production floor, Centennial Plastics: On January 30, 2017 and at times prior thereto an employee performed work above a 480 volt energized switch gear assembly. The employee was feeding metal fish tape into a three and a half inch diameter pipe approximately 36 inches above the energized buss. An effective means of insulation or other safe work practice was not used to protect the employee from accidental contact, direct or indirect, with the energized buss and the hazard of an arc flash.
