29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems. On or about January 6, 2017 at 1073 Winterton Road, Evans, Georgia: Employees were engaged in the tie in to an existing sanitary sewer stand pipe in a trench approximately 6'-4" deep. The employer did not ensure that a competent person inspected the trench for deficiencies and hazardous conditions such as, but not limited to: a) working on sloping terrain with the spoil pile placed on the uphill portion of the trench; b) conditions indicating the need for a protective system or sloping of the trench, including recent heavy rains, working at depths greater than 5-feet; c) conditions indicating the need for safe entry and egress; d) potential for the spoil pile rolling back into the trench.
