29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. a. Haviland Plastic Products Inc. located in Haviland Ohio: On or about November 16, 2016, the employer did not ensure the die and sheet rollers for machine #20 located in the production area were locked out and the heat dissipated from the equipment prior to employees working near the hot surfaces while removing the die.
