29 CFR 1910.268(j)(3): Vehicle-mounted elevating and rotating work platforms were operated with a conductive part of the equipment closer to exposed energized power lines than the clearances set forth in Table R-2 of this section: (a) On or about October 25, 2016, on the east side of FM 1461, an employee installing telecommunications strand operated a non-insulated, truck-mounted, articulating telescopic aerial lift closer than the minimum approach distance prescribed for an energized overhead power line with a phase-to phase voltage of approximately 24.9 kV.
