29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: On or about October 24, 2016, at the above addressed jobsite, the 5/8 eye bolts used to support the swing stage scaffold were not screwed fully into the drop-in anchors and the drop-in anchors were installed below the roof surface.
