29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a. On or about September 20, 2016, and times prior thereto, in the mill area, employees were exposed to electrical, thermal, and struck by hazards when servicing a STS Glue Table #2 without clear and specific procedures to control the potential release of hazardous energy. b. On or about September 20, 2016, and times prior thereto, in the mill area, employees were exposed to electrical, thermal, and struck by hazards when servicing a Dynamelt Glue Table without clear and specific procedures to control the potential release of hazardous energy. c. On or about September 20, 2016, and times prior thereto, in the mill area, employees were exposed to electrical and struck by hazards when servicing a CNC wood cutting machine without clear and specific procedures to control the potential release of hazardous energy. d. On or about September 20, 2016, and times prior thereto, in the mill area, employees were exposed to electrical and struck by hazards when servicing a wood Boring Machine without clear and specific procedures to control the potential release of hazardous energy. e. On or about September 20, 2016, and times prior thereto, in the mill area, employees were exposed to electrical and struck by hazards when servicing an Auto-Nailer without clear and specific procedures to control the potential release of hazardous energy.
