29 CFR 1926.1404(e)(1): Before a crew member goes to a location that is out of view of the operator and is either in, on, or under the equipment, or near the equipment (or load) where the crew member could be injured by movement of the equipment (or load), the crew member did not inform the operator that he/she is going to that location. During the assembly and disassembly of a DEMAG AC 500-1 Telescoping Boom Crane (serial number 84090), the operator was not informed when employees accessed areas on the crane that were out of the operator's view exposing employees to struck-by and crushed by hazards.
