29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) Worksite - The employer does not ensure that appropriate fall protection equipment or methods are in proper use. This violation was most recently observed at 1325 Chelsea Drive, Bethlehem, PA on June 22, 2016, when employees engaged in framing work were working on the second floor top plate, approximately 9 to 33 feet above a lower level, while having their vertical lifeline installed where the employee using the lifeline could fall in excess of six (6) feet or strike the lower level. The employer's ongoing obligation is to ensure that the necessary fall protection equipment is properly implemented for the duration of the fall hazard.
