29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware was not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: Instances including, but not limited to, the following: a) Job site, 140 Commerce Drive, Freedom, PA: On or about June 1, 2016, the employer did not ensure that appropriate equipment for energy control was provided prior to performing preventive maintenance work on the Make On/Pull On machine, company reference number 003507, in that air valve lockout devices were not provided.
