29 CFR 1910.146(d)(3): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations; a.) Terminal Ready Mix, Inc. located at 524 Colorado Ave. Lorain, OH; On or about May 8, 2016, the employer did not ensure the permit required confined space program was implemented prior to employees entering the 12 Yard Plant Cement Mixer. Employees are required to enter the plant mixer to chip out and replace the polyurethane liners and did not perform air monitoring or lock out the mixer prior to entry.
