29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers was not equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails: On or about 6/2/2016 on the in the equipment/chemical area of the facility located at 2434 West Broad Street in Athens, GA: Employees using the stairs leading to the upper mezzanine area were exposed to a fall hazard of up to 11 feet 3 inches to the lower level from the midrails missing.
