29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: At Simmons Food, Inc.: The employer failed to provide flame resistant clothing to production employees who were working with and handling combustible dust material such as, blood meal. On March 26, 2016 at approximately 11:45 pm an explosion occurred at the ProCal area of the establishment, located at 10700 South State Highway 43, Southwest City, Missouri 64863. Three employees were exposed to fire and explosion hazards when blood material was over dryed during the recycle stage of the process.
