29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws. a) Northwest Wall, Aluminum Damper Department, 450 Riverside Drive, Wyalusing, PA - A Stone Saw Co., Incorporated Stone Saw (Serial No. 0307) was powered with a strained electrical cable that did not allow the male end to sit flush with the upper outlet receptacle of a mounted receptacle box exposing employees to electric arc flash hazards, as observed on or about March 15, 2016.
