29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a) BIRDSBORO KOSHER FARMS FACILITY - During repair and service activities, employees performed servicing and maintenance operations on production equipment including but not limited to the AMFEC Mixer Model # 510, the chamber machine, flutie machine and frank-o-matic and the employees had not been provided authorized employee lock out tag out training, thereby exposing employees to serious injury, on or about March 2, 2016. Birdsboro Kosher Farms Corp. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 951711, citation number 1, item number 2a and was affirmed as a final order on October 03, 2014, with respect to a workplace located at 1100 Lincoln Road, Birdsboro, PA 19508. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
