29 CFR 1910.119(d)(3)(i)(B): The employer's piping and instrument diagrams (P&IDs) were not accurate and did not represent equipment that was existing and was part of the process. a) In the outdoor, underground propane tank area at 106 Lonczak Drive Chicopee, MA: On or about April 15, 2016, the employer's P&ID of the propane and endothermic gas piping, M-001, was not accurate in that the location of the emergency shut-off valve at the fence of the propane tank area was shown to be between the scrubber and the vaporizer and the shut-off was located between the scrubber and the propane storage tank. b) Along the Northwest exterior wall at 106 Lonczak Drive Chicopee, MA: On or about April 15, 2016, the P&ID did not show a controlled relay on the outside of the Northwest wall of the facility and a controlled relay was in place on the outside of the Northwest wall next to an overhead door which leads out to the propane storage tank area.
