29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing, or blocking of machines or equipment from energy sources: a) Fabrication department CNC Bender BD-01; On or about January 26, 2016 and at times prior thereto, the employer exposed employees to the hazardous energy (including electrical and pneumatic) on the machine in that the employer did not provide or make available energy isolation device(s) for employees to utilize when changing out the jigs on the bender in order to secure these hazardous energy sources. b) Fabrication department CNC Bender BD-03; On or about January 26, 2016 and at times prior thereto, the employer exposed employees to the hazardous energy (including electrical and pneumatic) on the machine in that the employer did not provide or make available energy isolation device(s) for employees to utilize when changing out the jigs on the bender in order to secure these hazardous energy sources. c) Fabrication department Endformer EF-03; On or about January 26, 2016 and at times prior thereto, the employer exposed employees to the hazardous energy (including electrical and pneumatic) on the machine in that the employer did not provide or make available energy isolation device(s) for employees to utilize when changing out the tubing on the endformer in order to secure these hazardous energy sources.
