29 CFR 1910.132(a): Protective equipment was not used when hazards capable of causing injury and impairment were encountered: (a) 120 85, LLC, at 10925 E. 120th Ave., Henderson, CO 80524: On and before January 19, 2016, the employer did not ensure that employees utilized personal fall protection when greasing the head pulley at the top of the conveyors. Employees on top of the conveyors were exposed to fall hazards of up to approximately 40 feet. This condition resulted in an employee falling approximately 40 feet to the ground below.
