29 CFR 1910.132(d)(1)(i): The employer did not assess the workplace to determine if hazards are present, or likely to be present, which necessitate the use of personal protective equipment (PPE): (a) Fill room mixing area - On or about 11/19/15, a mixer employee's face, head, torso, arms, legs, and feet were exposed to liquid splashes from chemicals such as, but not limited to, 93% sulfuric acid (pH of 0.3) and 45% potassium hydroxide (pH of 12 - 14) when transferring them from a bucket to a stainless steel mixing tank. The employer did not assess the workplace to determine if hazards were present which necessitate the use of personal protective equipment (PPE). ABATEMENT DOCUMENTATION REQUIRED
