29 CFR 1926.952(a)(1): In assigning an employee or a group of employees to perform a job, the employer did not provide the employee in charge of the job with all available information that relates to the determination of existing characteristics and conditions required by � 1926.950(d): a) 2998 Glenwood Drive, Camp Hill, Pa 17011 Substation - The employer failed to ensure that the existing characteristics and conditions of bay #4 was discussed with personnel assigned to work in bay #5, an employee received an electrical shock when the aerial work platform he was working from came too close to an energized transformer in bay #4 on or about September 28, 2015. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
