29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. On or about 25 September 2015, four employees were working from a job-site made wooden trash box on the forks of a rough terrain, telescoping boom forklift up to approximately 19 feet high without the employer ensuring that the workers were trained to identify the hazards of working from scaffolding, exposing the employees to a fall hazard.
