29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about September 3, 2015, at the Hydrodeoxygenator (HDO) Unit, the employer did not ensure that written lock out/tag out procedures contained specific steps and techniques for locking out Valve 5208 or the same type of valve (solenoid) including but not limited to identifying its dual energy sources (electricity and air), how to isolate the electrical power source and its location, how to isolate the air source and its location, how to dissipate any residual electrical and air energy, and how to verify all energy sources are isolated to prevent the release of highly hazardous chemicals (hydrogen gas).
