29 CFR 1910.1030(g)(2)(i): The employer shall train each employee with occupational exposure in accordance with the requirements of this section. Such training must be provided at no cost to the employee and during working hours. The employer shall institute a training program and ensure employee participation in the program. a. Located at 100 East Broadway Street in Jackson, Ohio: On or about July 27, 2015, untrained employees were assigned the task of cleaning up blood on a forklift and the concrete floor located in Freezer 6 after a forklift accident. 29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources. a. Located at 100 East Broadway Street in Jackson, Ohio: Authorized employees were not provided with an adequate number of locks in order to effectively lockout the extruder's main control panel and main screw drive disconnect, exposing employees to an amputation hazard.
