29 CFR 1926.850(a): Prior to permitting employees to start demolition operations, an engineering survey was not made, by a competent person, of the structure to determine the condition of the framing, floors and walls, and possibility of unplanned collapse of any portion of the structure. (a) Centimark Corporation, 319 Industrial Drive, Franklin, Ohio: On or about October 12, 2011, at the worksite located at 1820 McClain Road, Lima, Ohio, the employer did not conduct an engineering review of the structure before employees removed a section of concrete roof panels. Employees were exposed to a 65 feet fall hazard when a concrete roof panel collapsed while employees were working on the roof.
