29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a. Hearthside Food Solutions, LLC - McComb, Ohio: On or about April 21, 2015, the employer did not ensure that employees were protected from hazardous energy sources while removing jammed/excess dough from the Line #10 rotary cutter. The machine was not shut down and energy isolating devices were not used when employees were exposed to rotating parts associated with the cutter to remove excess dough. b. Hearthside Food Solutions, LLC - McComb, Ohio: On or about April 30, 2015, the employer did not ensure that employees were protected from hazardous energy sources while setting up the Line #10 cutter. The machine was not shut down and energy isolating devices were not used when employees were exposed to rotating part hazards to change the rotary cutter. c. Hearthside Food Solutions, LLC - McComb, Ohio: On or about May 20, 2015, the employer did not ensure that employees were protected from hazardous energy sources while setting up the Line #11 cutter. The machine was not shut down and energy isolating devices were not used when employees were exposed to rotating part hazards to change the rotary cutter. 29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a. Hearthside Food Solutions, LLC - McComb, Ohio: On or about April 21, 2015, the employer did not ensure employees who service equipment such as, but not limited to, the rotary cutter at Lines 10 and 11, were adequately trained in the skills required for the safe application, usage, and removal of energy controls. Employees that set-up the cutter and/or unjammed material from the cutter were not trained to isolate all energy sources during those tasks.
