29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A.) Dean Dairy Holdings, LLC, dba Dean Dairy Products Company, 1858 Oneida Lane, Sharpsville, PA 16150, on or about March 23, 2015 and times prior thereto: On or about March 23, 2015, an employee suffered an amputation on a Cherry-Burrell EPOCH II packaging machine (M/N: Q-7, S/N: 4157), also referred to as the quart filling machine, when performing servicing and maintenance activities that included, but were not limited to, clearing a jam / retrieving a faulty cardboard carton, in the machine. The machine contained moving parts including, but not limited to, a moving chain conveyer and an automatic suction paddle. The machine was not locked out prior to accessing the danger area of the moving parts when performing the servicing and maintenance activities.
