29 CFR 1910.333(b)(2)(ii)(A): Safe procedures for deenergizing circuits and equipment were not determined before circuits and equipment were denergized. On or about March 4, 2015 safe procedures were not determined prior to an employee de-energizing a circuit, and replacing a motor, exposing an employee to the hazards associated with electrical shock/ electrocution while working on a 480 volt circuit. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
