Text For Citation: 01 Item/Group: 001B Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries from unintended ram movement due to failure in air value or air supply: a) Press Area - No air pressure switch was installed on the incoming air supply for the clutch/brake, nor were tandem or dual air valves utilized to prevent increase in stopping time due to failure in the operating valve mechanism and thus exposing employees to injury where employees use a hands-in-die operation to feed powder and remove green part from the Baldwin 200 ton press. Press las used on or about February 21, 1992. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1. Install air pressure switch on incoming air supply line. 2. Install tarden or dual air valves on air clutch control system.
