29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a. On or about 01/20/2015, Morbark 6600 Wood Hog horizontal grinder - employees were exposed to a crush-in, struck-by and point of operation hazards, in that, lockout or tagout devices were not installed while replacing the grates, as required by the operators safety manual.
