Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to the employee in that: On or about 3/23/93 employees) operating a freight elevator, and/or riding a freight elevator were exposed to the hazard of falling into the elevator shaft and/or being crushed or being struck by the elevator car. a) Location: North and South side of elevator having 4 levels; 1st., 2nd., 3rd., and 4th. Floors; Among other methods one feasible and acceptable abnatement method to correct this hazard is to: 1. Equip each hoistway door with a hoistway door interlock device. 2. The interlock device shall hold the door in the locked position by means of gravity or by a restrained compression spring, or by both or by means of a positive linkage. 3. The interlock shall lock the door in the closed position before the elevator can be operated.
