29 CFR 1910.178(l)(3)(ii)(I): Powered industrial truck operators did not receive initial training in other unique or potentially hazardous conditions of the workplace that could affect safe operation: a) At the workplace, in the yard, on or about 10/23/14: Employees were loading 4,000 lb. panel-sections of pre-cast concrete onto a drop-bed trailer without having been trained on the hazards associated with unique types of pre-cast product relative to shape and size.
