29 CFR 1910.213(a)(9): All belts, pulleys, gears, shafts, and moving parts were not guarded as required: a) A & D Wood Products, Inc., Elida, Ohio: On or about September 5, 2014, the employer did not ensure that the drive belt and pulley system, which rotates the blade on the Irvington Machine Works manufactured cut-off saw, was guarded to prevent inadvertent contact while being operated. Employees were exposed to crushing and amputation hazards from ingoing nip points while engaged in wood cutting activities.
