29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was not isolated from the energy source and rendered inoperative: a) The employer does not ensure that a program consisting of energy control procedures, employee training and periodic inspections is established before an employee performs service or maintenance work on a powered industrial truck so that the truck is isolated from the energy source and rendered inoperative. This violation occurred on or about August 7, 2014, at a workplace located at 121 East Oregon Street, Dallas, Texas 75203; where an employee, who was working underneath a Halla, HLF25, forklift truck that was being supported by a hydraulic forklift jack, was exposed to the unexpected release of stored energy. The forklift truck was not isolated from falling and/or moving horizontally.
