29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: a. GreenStreet Coffee Roasters, Inc., Philadelphia, PA: Employees used a Mazzer Luigi Sri coffee grinder which was plugged into a wall-mounted electrical receptacle outlet missing a cover, exposing employees to electrical shock hazards. The employer did not ensure that the exposed energized parts were covered. Observed on and about July 24, 2014.
