29 CFR 1910.119(d)(3)(i)(B): The employer's piping and instrument diagrams were not accurate and did not represent equipment that was existing and was part of the process: In the Poly Vinyl Chloride (PVC) Unit at Formosa Plastics Corporation located in Point Comfort, Texas the employer did not ensure the piping and instrument diagrams (P&ID's) were updated and accurate. Discrepancies included, but were not limited to: a. Car sealed valves not shown on P&ID such as HV-402, HV-406 and HV-420 on Reactors PR-401A-J. b. P&ID drawings were not updated and still showed mark ups for MOC 850, removal of sight glass and the associated piping on PC-304 Recycle VCM gas scrubber c. P&ID drawings were not updated and still showed mark ups for MOC 784, modifications on piping systems on PV-306 Crude VCM Storage Tank d. P&ID drawings were not updated and still showed mark ups for MOC 723, modifications on piping systems for PC-301 Caustic Scrubber
