29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction, practical training, and evaluation of the operator's performance in the workplace: Work site: on or about June 24, 2014 and at times prior thereto, the employer failed to ensure that each employee had successfully completed training to operated the forklift. This condition exposed the employees struck-by and crush-by hazards.
