29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Drop out box of EAF exhaust system: On or about June 2, 2014 and at times prior thereto, the employer did not evaluate the need for, select, provide and require use of heat resistant personal protective equipment to protect employees from the thermal hazards associated with making contact with or being enveloped by hot EAF dust that becomes airborne while conducting the contracted services.
