29 CFR 1910.269(m)(2)(i): The system operator in charge of the lines or equipment and their means of disconnection, did not ensure all of the requirements of paragraph (m)(3) of this section were observed, in the order given: a. On or about March 14, 2014, the system operator did not ensure that all of the requirements of paragraph 29 CFR 1910.269(m)(3) were observed to de-energize a 23kV primary high voltage distribution power line which resulted in an electrical arc flash that injured two contracted employees.
