29 CFR 1910.132(a): Protective equipment was not used when hazards capable of causing injury and impairment to the body and extremities were encountered: (a) On or about November 20, 2013, for employees exposed to chemical hazards while conducting a non-routine pipe thawing task utilizing methanol where the exposure controls for personal protection to include, but not limited to chemical gloves, pants and jackets, were not adhered to as instructed on the material safety data sheet at an oil drilling site located 15 miles north on Hwy 22, Patterson Rig 268, Killdeer, North Dakota.
