(a) Lot 14 Westbury Subdivision, Hurricane, WV: An employee was installing a soffit utilizing a Stanley, model #TR150HL, staple gun on a new residential construction. The employer did not ensure that the employees were wearing protective eyewear, as observed on or about 11/05/2013. ABTEMENT CERTIFICATION REQUIRED
