29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about October 24, 2013 at worksite 1017 SW Drake Circle in Lees Summit, Missouri- Employees were laying and connecting a sanitary sewer pipe in a trench that was 8 to 14 feet in depth and were not instructed in the recognition and avoidance of the unsafe conditions while working in a trench. An employee was fatally injured in a trench that collapsed.
