29 CFR 1910.266(h)(2)(v): The undercut was not of a size so the tree would not split and would fall in the intended direction: a) At Gaynor Rd Logging Site: On or about October 8, 2013 and at time prior; the employer failed to ensure employees made correct notch cuts to allow a tree to fall in its intended direction exposing employees to struck-by and crush hazards. The angle cut should have been cut at a 45 degree angle and should have reached into the tree at least one fourth to one third of the way. This would have dictated which way the tree should fall.
