29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) On or about July 11, 2011, and at times prior, employees were exposed to cave-in hazards because the employer did not sufficiently instruct employees in the recognition of unsafe conditions and ensure that they were able to determine if the ground they stood on at the edge of an approximately 16 feet deep caisson (trench) had the structural integrity to support them. On July 11, 2011, an employee fell into a caisson when the ground he stood on next to the caisson opening collapsed because it had been undercut from sloughing and belling at the base.
