29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.: A.) Barry Smith Slate Roofing, Inc., PA Route 5 & Dutch Road, Fairview, PA 16415, on or about July 2, 2013 and times prior thereto: Employees were utilizing powered electrical equipment including, but not limited to, extension cords and a DeWalt DW359 7.25 inch circular saw. Ground fault circuit interrupters and/or an assured equipment grounding program were not being used in conjunction with the circular saw.
