29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons can accidentally walk were not guarded by a floor hole cover of standard strength and construction: On or about April 29, 2009, and at times prior thereto, at the west side of the tank wash bay area, the employer did not take precautions to ensure that employees walking/working in the area did not accidentally trip, slip or fall into the floor holes where metal grates were absent.
