29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. On or about March 22, 2013, and at times prior thereto, the employer did not develop or implement written lockout/tagout procedures for each type of equipment or machine located within the welding shop. Employees performing maintenance work on the equipment such as but not limited to the Niagara metal press, and the Dayton 1 ton overhead crane were exposed to electrical shock and amputation hazards.
