29 CFR 1926.451(g)(4)(i): Guardrail systems was not installed along all open sides and ends of platforms. On or about March 13, 2013, a guardrail system were not installed on open sides and ends of a scaffold platform erected at an approximately height of 12 feet from the ground before it was released to use by employees. These conditions exposed employees to fall hazards.
