29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: (a) Beaver Builders and Excavation, Inc. @ 4251 Grove St., Denver, CO 80211: On and before 1/16/13, the employer did not provide employees with information provided in Subpart P Excavations. An employee conducted residential sewer tap repair while inside a trench, which measured approximately 8 ft. x 3 ft. x 8 ft. deep and was not trained to recognize or anticipate hazards associated with working inside a trench. This condition exposed an employee to stuck-by hazards.
